Developmental Screening and Evaluation

For children under 5:

If you have concerns about an individual child, you can contact Early Intervention/ Early Childhood Special Education programs in your area for screening and/or evaluation to find out whether the child is eligible for state-funded services. Anyone can make a referral; the program will then contact the parents to determine whether they want to have the child evaluated.

If you would like to host a general screening at your clinic, preschool, or other program, please contact Dr. Lennert at 503-539-4654 or at drlennert@connectionsps.com to talk about options.

If you are interested in an independent evaluation of a child's developmental, emotional, or learning needs, contact Dr. Lennert at 503-539-4654 or at drlennert@connectionsps.com for more information or to schedule an appointment.

For children over 5:

If you suspect learning disabilities or other disabilities for a school-age child, contact your local school to request a meeting to talk about your options. Schools are required to identify children who need specialized instruction in order to make progress. For children with disabilities, schools work with parents to develop an Individualized Education Plan to address the child's needs.

For an independent evaluation of possible learning disabilities or mental health problems, contact Dr. Lennert at 503-539-4654 or at drlennert@connectionsps.com for more information or to schedule an appointment.
